Please fill out this customization form including mascot, colors (hex codes if available) and sport.  Please include anything you would like to add or take away.  If there are major changes, please contact me first, but if they are minor, then the form should be sufficient!:)

Our Custom Etsy Mod allows up to 2 modifications/changes including colors, logos, and more of existing Etsy listings. 

PNG's are 300dpi and are sent as finished files and are not for editing.
